The Jotform Sign platform is easy and intuitive to use. Information input required:
Entering a valid email address is the first step. Upon completion, an email and a signed copy will be sent to that email address. If a confirmation email is not received then there was an error in the email
address given. The Declarations will be automatically uploaded from Jotform on to an IDBF account. They DO NOT need to be printed out.
PLEASE COMPLETE ALL DECLARATIONS BY AUGUST 31, 2024 AT THE LATEST
